Deliberate Safety Control Defeat

A subject disables, bypasses, overrides, falsifies, or prevents the operation of a safety control, resulting in an unsafe condition, exposure, near miss, injury, environmental impact, or damage to equipment.

 

Safety controls may be physical, procedural, mechanical, digital, or administrative. Examples include bypassing an interlock, falsifying an inspection, suppressing an alarm, overriding a maintenance restriction, or returning unsafe equipment to service.

 

The observable harm may include actual injury or damage, but it should not require it. A documented unsafe condition or near miss caused by the subject’s act is itself a concrete adverse outcome.
